Identification of minimal hepatic encephalopathy based on dynamic functional connectivity

چکیده
To investigate whether dynamic functional connectivity (DFC) metrics can better identify minimal hepatic encephalopathy (MHE) patients from cirrhotic without any (noHE) and healthy controls (HCs). Resting-state MRI data were acquired 62 with cirrhosis (MHE, n?=?30; noHE, n?=?32) 41 HCs. We used the sliding time window approach analysis to extract time-varying properties of brain connectivity. Three DFC characteristics (i.e., strength, stability, variability) calculated. For comparison, we also calculated static (SFC). A linear support vector machine was differentiate MHE noHE HCs using SFC as classification features. The leave-one-out cross-validation method estimate performance. strength (DFC-Dstrength) achieved best accuracy (MHE vs. 72.5%; HCs, 84%; 88%) compared other Compared features, accuracies DFC-Dstrength feature improved by 10.5%, 8%, 14% for HC, respectively. Based on DFC-Dstrength, seven nodes identified most discriminant features classify including left inferior parietal lobule, supramarginal gyrus, calcarine, superior frontal cerebellum, right postcentral insula. In summary, have a higher in identifying patients. Our findings suggest usefulness capturing neural processes disease-related biomarkers important identification.

Role of local and distant functional connectivity density in the development of minimal hepatic encephalopathy
The progression of functional connectivity (FC) patterns from non-hepatic encephalopathy (non-HE) to minimal HE (MHE) is not well known. This resting-state functional magnetic resonance imaging (rs-fMRI) study investigated the evolution of intrinsic FC patterns from non-HE to MHE. A total of 103 cirrhotic patients (MHE, n = 34 and non-HE, n = 69) and 103 healthy controls underwent rs-fMRI scann...

Minimal (subclinical) encephalopathy reflects alterations in cognitive fu...

Minimal hepatic encephalopathy (MHE) is the mildest form of spectrum of hepatic encephalopathy (HE). Patients with MHE have no recognizable clinical symptoms of HE but have mild cognitive and psychomotor deficits. The prevalence of MHE is high in patients with cirrhosis of liver and varies between 30% and 84%; it is higher in patients with poor liver function. The diagnostic criteria for MHE ha...

Hepatic encephalopathy (HE) is a neuropsychiatric disorder that may accompany either acute or chronic liver disease. It is defined as a disturbance of central nervous system function due to hepatic insufficiency and includes a large spectrum of clinical manifestations such as decreased intellectual function, personality disorders, alterations in level of consciousness and neuromuscular dysfunct...
